Supporting People with Disabilities—and the Families Who Walk Beside Them
We believe in the sacredness of daily life.
JMJ Community Partners is a Medicaid-approved provider agency specializing in Colorado Medicaid disability services, delivering in-home and community-based support for individuals with intellectual and developmental disabilities. We ground our care in relationship, dignity, and the quiet rhythms of everyday life—where true support takes root.
We intentionally stay small so we can work closely with each family, offering hands-on guidance and truly personalized care. We honor each person’s unique story, needs, and goals through thoughtful, individualized support that respects the dignity of daily life.
At JMJ, many of us have walked this road ourselves. We understand how overwhelming the system can feel, and we walk alongside you every step of the way.

Why JMJ?
Personalized Care Planning: We don’t just check boxes—we listen. Our care plans are co-created with individuals and families to reflect what truly matters: your goals, your challenges, your joys. Every plan is built around your voice, your values, and your dreams for the future.
Home Visits & Frequent Check-Ins: The best way to understand someone’s life is to step into it. Through regular home visits and personal check-ins, we stay closely connected. This builds trust, helps us adjust services quickly, and verfies no one ever feels forgotten.
Detailed Oversight that Cares: We treat oversight as a form of care and stewardship. We monitor services closely, address concerns swiftly, and maintain open communication with case management agencies and families. Every detail matters—because people matter.
Support for the Whole Family: From waiver education and self-advocacy to workshops on Colorado Medicaid family caregiver programs, we equip families to navigate the disability system with confidence. We focus on helping families understand their options in Colorado as a paid family caregiver for intellectually disabled loved ones — because sometimes the best support comes from someone who already loves them.
Advocacy At the Case Management and State Level: We bring real stories and real solutions to conversations with HCPF and local Case Management Agencies. Advocacy isn’t an afterthought—it’s built into everything we do.
Efficient Planning. Better Outcomes: By truly understanding each person’s needs, we verify supports are aligned, purposeful, and never wasteful. We focus your resources where they matter most—for less stress and more meaningful progress.
The Heart of the Mission: We believe every person is uniquely made, holding sacred worth and the capacity to live a full, connected life. Inspired by the beauty found in quiet care and enduring presence, we serve to build lives marked by dignity, joy, and belonging.
